The diabetes centre houses the specialist diabetes team and diabetes support services including diabetes consultants, diabetes specialist nurses, specialist chiropody, dietetics, diabetes support workers, the district eye screening office and the diabetes register office. We are here to offer an accessible and efficient specialist service for people with diabetes and primary and community health care teams. People with diabetes should use their general practice teams as the first line of call, they may refer you here for special help but we will always help a patient or their carers if there is an urgent need. The Clinic Nursing team will see you prior to your visit with the clinic doctors to ensure that everything is prepared. Please bring your urine sample, a complete list of you medications, your diabetes blood test record book and your diabetes meter. If it has been a year since you had your eyes tested – we will do the eye test – do not drive on that day. Blood tests can be done here as needed – any weekday morning. We will offer advice as required or point you in the right direction for any help you need. We will organise any special test that are requested.

All patients attending the Wolverhampton Diabetes Centre have a named consultant who is responsible for your overall care. Consultants and their teams will usually see those patients who have complex or difficult problems. The consultants will make them selves available to give advice to general practitioners, practice nurses and other health care workers as required.

The Diabetes Nursing Team are trained, experienced specialist practitioners offering education and treatment together with wider health and social needs assessments for people with diabetes. Referrals are accepted from all other health professionals seeking support for their patients. People with diabetes and / or their carers can refer themselves directly for help and advice, especially if they have a pressing or urgent need.

The diabetes centre foot clinics are mainly for patients with serious high risk foot problems. General chiropody care, if needed, is delivered at local health centres. All people with diabetes should be referred here by their local health team in case of serious problems or emergencies. However, if you have a new foot problem and are concerned about your feet, we will be happy to advise. We also deliver foot education sessions for newly diagnosed patients and an Asian foot education session is run for those with English language difficulties.
There are 2 podiatrists at the centre, Edward Spencer and Sue Huddart.

Our role is to help support and develop an integrated diabetes health care service across the whole of Wolverhampton, enabling effective delivery and implementation of diabetes care to all people with diabetes including those of the BME community. We particularly wish to support those with difficulty in accessing diabetes care for what ever reason. Our role is to identify, and subsequently minimise, organisational, social, cultural, language and personal barriers faced by people in Wolverhampton when using NHS services for diabetes. Individuals and groups should contact us for help if they are having any such difficulties. We are also keen to support any community activities that promote diabetes health.

The Diabetes Information System (Diabetes Register) is a database containing data about people with diabetes. It is used to monitor and audit the quality of the Diabetes Service that is provided across Wolverhampton to ensure effective care for patients. It holds information such as diabetes type and treatment, complications, blood results and annual review data. This information is confidential and is used only to examine and improve the service that patients’ receive.

Everybody with diabetes should have an eye examination at least annually. The Wolverhampton Eye Screening programme was developed to provide a full annual eye examination for patients in Wolverhampton who don’t already attend the Wolverhampton Diabetes Centre or Eye Infirmary. Patients can be referred by their GP to any one of the accredited optometrists (opticians). If the patient is already under the care of one of our accredited optometrists, they can be placed directly into the programme. Information regarding the examination is fed back to us at the Diabetes Centre and entered into the Diabetes Register. If any important findings are seen during the examination, an appointment will be automatically sent out to attend a special eye clinic at the Wolverhampton Diabetes Centre. Patients who are found to have no complications will be recalled to the Optometrist in 12 months time. |
